On line one print your name with title as you would like it to appear on the center plate of your championship belt. For example: Shihan Tim Scott, RET COL Marvin Smith, Evangelist Pearl Hall, etc.
On line two check the box of your category, either Martial Arts or Community Leader.
On line three provide your best phone number in case follow-up is required.
